By submitting this form, you are asking that School of Imagination reserve a place for your child for Fall 2007. If you require enrollment earlier, we will do our best to accomodate you.
This deposit is not refundable, except in emergency situations. After we have received your signed application and check, we will send all enrollment forms.
Enrollment forms and immunization records are due BEFORE the first day of the class. As a courtesy, we will remind you once. If received after the first day of class, we will give up your child’s spot, unless other arrangements have been made.
